Case Management Services
Case Management Services provide personalized, client-centred, comprehensive, one to one support for eligible newcomers who need additional assistance to successfully settle and build independence in Canada.
Through this program, clients work closely with experienced Case Managers who help identify needs, set goals, and access essential services. Support includes regular check-ins, accompaniment to appointments, and outreach or home visits, depending on individual circumstances.

Program Duration
- Up to 12–18 months of individualized support, based on client needs
Services Offered
- Individualized action and goal setting plans
- Referrals to housing, employment, health, education, and community resources
- Outreach support, accompaniment, and home visits when required

Services are available in: Amharic, Arabic, Dari, English, Farsi, Kurdish, Tigrigna, Turkish, and Vietnamese
